Axis Duo MD provides non-emergency medical services through telemedicine.
Use of the Axis Duo MD website or scheduling platform does not establish a physician-patient relationship. A physician-patient relationship is established only when an Axis Duo MD physician accepts and evaluates you as a patient through an appropriate clinical encounter.
You agree to provide accurate and complete information regarding your identity, physical location, medical history, medications, allergies, symptoms, and other information relevant to your care.
You must be physically located in a state where your treating Axis Duo MD physician is licensed at the time of your appointment.
Axis Duo MD does not provide emergency medical care. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
Telemedicine may not be appropriate for every medical condition. Your physician may recommend in-person care, laboratory testing, imaging, urgent care, emergency care, or evaluation by another healthcare professional.
Prescriptions are provided solely at the treating physician’s medical discretion and only when medically appropriate. Payment for a visit does not guarantee a prescription, medication refill, laboratory order, diagnosis, referral, or specific treatment.
Axis Duo MD does not prescribe controlled substances.

You agree not to misuse the website, patient portal, scheduling system, or telemedicine services, including attempting to obtain medications through false or misleading information.
