We understand that plans can change. If you are unable to attend your scheduled appointment, please cancel or reschedule as soon as possible.
Appointments canceled at least 24 hours before the scheduled appointment time may be rescheduled without an additional visit fee.
Appointments canceled less than 24 hours before the scheduled appointment time and missed appointments are non-refundable.
Eligible appointments may be rescheduled within 30 days of the original appointment date.
Patients who arrive late may receive a shortened visit or may need to reschedule depending on physician availability.
If Axis Duo MD is unable to complete your scheduled visit because of a scheduling, technical, or provider-related issue on our end, we will work with you to reschedule the appointment or provide another appropriate resolution.
