Telemedicine allows you and your physician to communicate through secure electronic technology while you are in different locations.
By using Axis Duo MD telemedicine services, you understand and agree that:
• Telemedicine has limitations compared with an in-person medical examination.
• Your physician may determine that your condition cannot be safely evaluated or treated virtually.
• You may be advised to obtain laboratory testing, imaging, an in-person examination, urgent care, emergency care, or evaluation by another healthcare professional.
• Technical difficulties may occasionally interrupt or delay your visit.
• You are responsible for providing complete and accurate information about your medical history, medications, allergies, symptoms, and current physical location.
• You must be physically located in a state where your treating physician is licensed at the time of your appointment.
• Prescriptions are provided only when medically appropriate following physician evaluation.
• A telemedicine visit does not guarantee a prescription, medication refill, laboratory order, diagnosis, or specific treatment.
• Axis Duo MD does not prescribe controlled substances.
You may choose to stop or withdraw your consent to telemedicine at any time before or during the visit. Withdrawal of consent does not affect care already provided.
By proceeding with your appointment, you acknowledge that you understand the benefits, risks, and limitations of telemedicine and voluntarily consent to receiving care through Axis Duo MD.
